What is an APD provider?

APD (Agency for Persons with Disabilities) is Florida’s state agency that helps people with developmental disabilities — intellectual disability, autism, cerebral palsy, spina bifida, Down syndrome, and more — and their families get the supports they need. Providers like One 2 One Transition are trusted partners who deliver those supports for adults 18 and older: in the home, in the community, and in everyday life.

How does an individualized plan work?

Support starts with a conversation about the person’s daily life, abilities, and goals. From there, a plan is built around them — which services are involved, who provides them, and how progress is measured. Families and support teams stay involved all the way through.

What should I prepare for a first call?

Nothing formal — but a few things help us. A general sense of the person’s routine, any diagnoses and waivers in place, who else is on their support team, and the two or three things you hope support will change most.
