My name is Fabiola Fanfan, and I am the proud Owner and Operator of One 2 One Transition.
Helping individuals with disabilities is not just my profession — it is deeply personal to me and very close to my heart. I truly believe that everyone deserves to feel seen, respected, supported, and cared for with compassion.
When you come to One 2 One Transition, I want you to know that you are more than a client. My hope is that you feel like part of a family — a family that is here to listen, encourage you, celebrate your progress, and stand beside you through every step of your journey.
Every person has their own story, their own needs, and their own dreams. It is my privilege to be a part of that journey and to provide support that brings comfort, dignity, independence, and hope.
